01About MED-WECARE
Care that starts in the community and is measured by its outcomes.
Founded in 2021, MED-WECARE is a Nigerian healthcare organization working at the intersection of community health, preventive care, health education, digital health and research.
Many Nigerians live with conditions such as hypertension that go undiagnosed until complications arise. We go to where people are — markets, churches, town halls and rural communities — to screen, educate and connect them to care, and we follow up with digital tools that keep patients from falling through the cracks.

Our outreaches bring screening and education directly into communities, then link people to the care they need.

05Digital Hypertension Registry
Turning one-time screenings into continuous care.
Hypertension is often silent and lifelong. Our digital registry keeps track of patients beyond the first encounter, so that care continues after the outreach ends.
